This is a remote hybrid position.
*MDS Coordinator Requirements:*
* Applicant must possess strong communication, clinical, and organizational skills.
* MDS experience preferred.
* Current/active RN licensure required.
*MDS Coordinator Job Duties:*
* Transcribe and take appropriate action on physician’s orders as directed including but not limited to Care Plan changes.
* Assist with performing all admitting and discharge assessments and complete all other necessary paperwork.
* Help monitor all aspects of patient care.
* Initiate requests for consultation or referral.
Respond to requests from the resident, physician, or nursing staff.
* Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained.
* Coordinate and prepare MDS assessments for all residents.
* Oversee the transmission of MDS resident assessments.
* Prepare, monitor, and update assessments, Care Area Assessments, and Care Plans, as necessary.
* Participate in Care Plan meetings with residents and family members.
* Be aware of residents’ exit to and reentry from hospitals.
